A Note to Former Patients and Friends of Dr. Vichai Duangjak

First, I would like to inform my former patients who have received letters from me that the phone number for Drs. Cameron and Andreani listed in the letters was incorrect. Please note that the correct number is 440-285-0828.

Second, please note that my medical records will be kept at a storage facility in Chesterland (except for those transferred to Drs. Cameron and Andreani). In order to receive a copy, you will need to provide what they require, including any necessary consents and paying a fee.

Finally, I would like to express my gratitude to all my office staff for their many years of hard work and dedication, to the nurses and other staff at Geauga Hospital and Middlefield Birthing Center for their support.
