Senior Adult Assessment
University Hospitals Geauga Medical Center, in partnership with the Geauga County Department on Aging, offers a proactive course of action for seniors who may be at risk because of undiagnosed illness and home safety issues. The hospitals Senior Adult Assessment Program provides a comprehensive evaluation of an older persons medical and social situation in one or two convenient appointments.
The one-stop assessment at UH Geauga Medical Center takes three to four hours and includes consultations with an internist, physical therapist, geriatric psychiatrist, pharmacist and social worker. Any additional or required tests are ordered at that time. Following the appointment, an occupational therapist is scheduled to visit the patients home and evaluate it for safety and accessibility concerns.
While the Senior Adult Assessment and any associated tests are not free of charge, the Geauga County Department on Aging has been underwriting the cost of the consultations for the past two years.
