PUBLIC NOTICE
Eco-Site, Inc. is proposing to install a monopole at 11135 Heath Road, Chester Township, OH (41-34-8.5N, 81-189-2.9W. The height of the tower will be 60.7 meters above ground level (297.2 meters above mean sea level). The tower is anticipated to have no lights. Interested persons may review the application for this project at www.fcc.gov/asr/applications by entering Antenna Structure Registration (Form 854) file no. A1053512 and may raise environmental concerns about the project under the National Environmental Policy Act rules of the Federal Communications Commission, 47 CFR §1.1307, by notifying the FCC of the specific reasons that the action may have a significant impact on the quality of the human environment. Requests for Environmental Review must be filed within 30 days of the date that notice of the project is published on the FCC’s website and may only raise environmental concerns. The FCC strongly encourages interested parties to file Requests for Environmental Review online at www.fcc.gov/asr/environmentalrequest, but they may be filed with a paper copy by mailing the Request to FCC Requests for Environmental Review, Attn: Ramon Williams, 445 12th Street SW, Washington, DC 20554. An ordinance authorizing the City Manager to enter into a contract with Medical Mutual beginning January 1, 2017 for employee group medical insurance, $583.24/month for single coverage, $1,283.11/month for employee and spouse, $975.67/month for employee and children and $1,802.20/month for family.

An ordinance authorizing the City Manager to renew the contract with Guardian for group dental, vision and life insurance beginning January 1, 2017, for dental coverage in the amount of $33.32/month for single coverage and $91.41/month for family coverage; for vision coverage, $4.32/month for single coverage and $9.30/month for family coverage; and for life insurance, $0.123/month per $1,000 for salary/wages for life and $0.018/month per $1,000 of salary/wages for accidental death and dismemberment, with a maximum benefit of $150,000 until age 65.

An ordinance establishing the maximum amount payable by the City of Chardon for employee medical insurance: the maximum the City will pay is $524.92/month for single coverage, $1,154.80/month for employee/ spouse coverage, $887.10/month for employee/child(ren) coverage and $1,621.98/month for family coverage for medical insurance; and for dental insurance $29.99/month for single coverage and $82.27/month for family coverage..
